Buprenorphine Practitioner Verification for Pharmacists

The Substance Abuse and Mental Health Services Administration (SAMHSA) has put together a database to allow pharmacists to verify a physician’s DATA waiver. Pharmacists are able to search using the physician’s last name and DEA registration number to verify.

In order to help the public better understand what buprenorphine is and it’s importance in medication-assisted treatment (MAT), the Substance Abuse and Mental Health Services Administration (SAMHSA) has put together a webpage providing an overview. The page covers buprenorphine basics including how the drug works, it’s side effects, misuse potential and notes about safety, treatment and training.

Appointed Pharmacy Consent Form

The Boston Medical Center's Office Based Addiction Treatment Training and Technical Assistance Program offers a sample consent form specific to pharmacies. This includes federally mandated items as well as additional items that are practice specific to their program.
